Cleanup watchlist preference usage

In general, the web UI does a check of the watchlist preferences along
the lines of "watch if watchdefault, or if watchcreations and the title
doesn't exist". So there's no way to have it watch edits but not
creations. Make the API behavior match this.

For API action=protect&watchlist=preferences, we want to use
'watchdefault' always to match the behavior of the web UI.

For Special:Upload, the code is all there to do a "watch if
watchdefault, or if watchcreations and the file doesn't exist". But for
some reason that code wasn't being used in favor of just using
watchcreations all the time. Fix that, too. And have the API use that
instead of checking if the file page exists.

filebackend: improved "adviseStat" performance

* Use the normal page size instead of limiting it way down to the cache
  size. Track the stat information in the pages and load into into the
  stat cache as entries are accessed. This should also be less prone to
  evictions causing HEAD requests (or memcached hits).
* Also bumped CACHE_CHEAP_SIZE up to 500.

Support CIDR ranges in $wgSquidServersNoPurge

Use IP::isInRange() in wfIsConfiguredProxy() to allow matching against
CIDR entries in $wgSquidServersNoPurge. This will allow maintainers of
large networks to whitelist contiguous blocks of IPv4 and/or IPv6
addresses as trusted X-Forwarded-For providers.

This change also makes a small change to
WebRequestTest::testGetIpLackOfRemoteAddrThrowAnException() which was
failing under some configurations due to non-default globals
configuration.

jquery.spinner:
* While at it, updated documentation to be included in our
  jsduck index, similar to jquery.localize.

jsduck/external.js:
* Added @static to jQuery.ajax which was missing, it showed up
  between instance methods like jQuery#injectSpinner.

The only module left using mediawiki.legacy.wikibits is
mediawiki.legacy.ajax which remains in tact for now.
